Making an Appointment
To make an appointment with Dr. Wyatt, please have the following available when calling.
Patient's full name
Patient's social security number
Patient's birth date
Patient's insurance card
Patient's main reason for visit
Desired appointment date and time
Referral information if applicable
Be Sure to Bring the Following to Your Appointment
Photo identification
Insurance identification card, if applicable

Late Arrivals
We understand that people are busy and things can happen to delay your arrival; therefore, please make every attempt to arrive for your appointment at least 15 minutes in advance. If you arrive late, we will make every attempt to accommodate you. However, we may need to reschedule or delay your appointment if you arrive after the start of the next patient's appointment.
Cancellations
Cancellations must be made 24 hours in advance. Because dermatology appointments are in high demand, patients can be forced to wait many weeks or even months for an appointment. Cancelling 24 hours in advance allows us to offer your appointment to another patient.
If you do not cancel your appointment 24 hours in advance we will apply a $30.00 assesment to you account, payable upon your next office visit.
Insurance and Payments
We require payment of all office charges at the time of service. We accept Visa, MasterCard, American Express, Discover, Cash, and Health Savings Account debit cards. We are happy to bill your insurance carrier provided we are a contracted provider with them, and you provide us with a valid insurance card. If you require a referral, it is your responsibility to obtain the referral from the referring physician and present it at your appointment. You are responsible for all co-payments, co-insurance, deductibles, and any unpaid or denied services not covered by your insurance.
Identification
For your protection, we require valid photo identification at each office visit.

We will verify the following information at the time of each appointment.

Keeping this information up to date allows us to provide you with better service as well as to protect your private health information.
